It is the goal of the Luzerne-Wyoming Counties MHDS Intake Department to efficiently determine an individual’s eligibility for Supports Coordination, and to answer general questions regarding mental health and intellectual disability services in Luzerne and Wyoming Counties.
An individual may be eligible for Supports Coordination through a diagnosis of an Intellectual Disability, Autism Spectrum Disorder, or a Developmental Disability (prior to age 9). It is the job of the Intake Department to obtain documentation of appropriate cognitive, autism or adaptive testing to be reviewed by our psychologist in order to determine one’s eligibility. Those who are eligible, are assigned a Supports Coordinator who can locate, coordinate, and monitor community services for individuals with Intellectual Disabilities and Autism.
