• Serving ages birth and beyond

• Identify an individual’s vision for future and supports services needed to achieve their life plan

• Locate—community life activities, supports services, person driven planning

• Connect-facilitate connections to personal communities, activities and services

• Coordinate- supporting individuals in their effort to participate their community’s life

• Monitor-insure that community connections and systems' services meet desired goals and needs

• Advocate-promote opportunity to live, love, work, play and follow life’s dreams
