NOTICE: If you had signed up to receive a mail ballot for the upcoming election and no longer desire to vote by mail, you may request for your ballot request to be canceled by completing and returning this REQUEST TO CANCEL MY BALLOT REQUEST form.
Only the voter may complete their own form. After completing and signing the form, you must send it to the Elections office. The address is on page 2 of the form.
