Death Certificates: Individuals seeking information on how to obtain a copy of a Death Certificate may do so by calling the office at (570) 825-1664. We can provide you with an application. You may also order on-liine by visiting the state website: www.health.state.pa.us/vitalrecords.

 

Cremation Permits: Funeral directors in need of a Cremation Permit, may do so by faxing a completed copy of the Death Certificate to (570) 820-6319.

 

Calling a Coroner for the Expected Death of a Terminally Ill Person: If you need a coroner to come to your home or place of death of terminally ill individual who has expired of natural and expected causes, call the Luzerne County Communications Center at 911. A deputy coroner will be dispatched to the location of the death. You can also call your local police department and they will dispatch a police officer that will respond to the site and call the coroner's office. In all cases of unexpected or unexplained death of any individual, call 911 immediately.

 

Procedures for Unexpected Deaths: Call 911 immediately and request emergency medical services and police assistance.

 

Requests for the Coroner for Speaking Events: You may request the coroner to public speak at your event by written request to Coroner's Office, Penn Place Bldg., Suite 201, 20 North Pennsylvania Avenue, Wiles-Barre, PA 18701. You may also email your request to wallacm1@luzernecounty.org or call (570) 825-1664 directly between the hours of 8:30 a.m. and 4:30 p.m. on weekdays.

 

Mass Casualty Incidents: Call 911 immediately.
